PROCESS
The applicant needs to
send a letter that includes
the following information:
1. Drug and Alcohol History
– What drugs/alcohol
they use and how long
they have been using.
2. Legal and Criminal
History – What the
crime was and when it
occurred (if any). Include
whether or not they’re
on probation/parole and/or
whether they have a case
pending.
3. Medical and Psychiatric
History – List medical/psychiatric
history and detail all
past treatment (inpatient
or outpatient). Also detail
any prescribed medications
in the past or at present
and the reasons for which
they are prescribed.
4. Brief Summary –
Why they think they need
long-term treatment. (Synergy
is a one- year program.)
5. Include name, address,
phone number, and a convenient
time when we may call.
