Synergy's
Transitional Living Center
houses
15 men and is located
in an apartment building
at 1157 Peabody in midtown
Memphis. The facility
is conveniently located
to public transportation
and mainstream resource
offices. Synergy Transitional
Living Center combines
stable housing and supportive
services to help men with
drug and/or alcohol dependence
move toward better health,
personal and financial
independence, and social
reintegration. Upon graduation
of Synergy’s one
year treatment program,
many men do not have a
stable living environment
to return to. These men
are encouraged to reside
at least one year at our
transitional facility,
and may stay up to two
years if necessary, with
the aim of securing permanent,
stable housing after their
stay. During this time,
Synergy provides a carefully
designed and customized
program of services for
each resident . A full-time
case manager coordinates
most services, which include
substance abuse treatment,
health assessment and
medical referrals, personal
and “life”
skills training, and guidance
related to educational
opportunities, job training
and employment. The case
manager works with each
resident to evaluate needs,
develop short and long-term
goals, identify and develop
appropriate resources
and services, and provide
personal counseling.
